What is Medicare Part D?
Medicare Part D is the part of Medicare that helps cover the cost of prescription drugs. These plans are offered by private insurance companies approved by Medicare. You can get Part D coverage either through a standalone Prescription Drug Plan (PDP) that works alongside Original Medicare, or as part of a Medicare Advantage plan (MAPD). We’re here to help you make sense of it all.
What Does Medicare Part D Cover?
Each Part D plan has its own list of covered drugs, called a formulary. While formularies vary from plan to plan, they must all cover a range of drugs in the most common categories.
- Prescription Drugs: Covers both generic and brand-name prescription drugs. It’s essential to check a plan’s formulary to ensure your specific medications are included.
- Vaccines: Covers most commercially available vaccines, like the shingles shot, to help you stay healthy.
- Formulary Tiers: Plans group drugs into different “tiers.” Drugs in lower tiers (like generics) typically have lower copayments, while drugs in higher tiers (like specialty drugs) cost more.
Who Is Eligible for Medicare Part D?
To be eligible to enroll in a Medicare Part D plan, you must meet a simple requirement:
- You must be enrolled in either Medicare Part A or Medicare Part B.
You don’t need to have both Parts A and B to join a drug plan. As long as you have one or the other and live in the plan’s service area, you can enroll. Enrollment is optional, but if you don’t sign up when you’re first eligible, you could face a permanent late enrollment penalty if you decide to join later.
What Are the Costs for Medicare Part D?
The costs for Part D plans can vary, so it is important to compare them based on the drugs you take.
- Monthly Premium: You’ll pay a monthly premium to the private insurance company that provides your plan. This amount varies by plan and can be higher depending on your income.
- Annual Deductible: Many plans have an annual deductible, which is the amount you must pay for your prescriptions before your plan starts to pay. The maximum deductible allowed in 2024 is $545.
- Copayments or Coinsurance: After you meet your deductible, you will pay a copayment (a flat fee) or coinsurance (a percentage of the cost) for each prescription you fill.
- Coverage Gap (Donut Hole): If your total drug costs reach a certain limit, you’ll enter the coverage gap. In this phase, you will pay a percentage of the cost for your drugs until you reach the out-of-pocket spending limit.

Frequently Asked Questions
What happens if I don't enroll in a Part D plan when I'm first eligible?
If you don’t sign up for a Part D plan when you first become eligible and don’t have other creditable prescription drug coverage, you may have to pay a late enrollment penalty. This penalty is a permanent addition to your monthly Part D premium for as long as you have coverage.
How do I choose the right Part D plan?
The best plan for you depends on the medications you take. You should compare plans based on their formularies, premiums, deductibles, and the copayments for your specific drugs. We can help you use Medicare’s tools to compare plans and find the one that is most cost-effective for your situation.
Can I change my Part D plan?
Yes. You can change your Part D plan every year during the Annual Open Enrollment Period, which runs from October 15 to December 7. This is a great opportunity to review your coverage and make sure your plan still meets your needs for the upcoming year.
